51 y-o male with unruptured 5mm (3.5 mm neck) A1 aneurysm. The curvature of the ICA and A1 would have caused a Neuroform stent to splay open at the neck of the aneurysm resulting in no effective protection. Play Video (WMV 1.1M) 4 x 7 MTI HyperForm balloon and Cordis Prowler 14 in place HydroCoil 10 3 x 4 coil placed View Image Angio prior to coil detachment Play Video (WMV 161K) Angio post coil detachment Play Video (WMV 153K) Attempted insertion of HydroCoil 2 x 6 coil caused protrusion of coil loops. 2nd coil withdrawn. Protrusion of loops from rearrangement of first coil confirmed. View Image Prowler 14 exchanged for Prowler Plus to allow passage of Alligator to remove coil. Coil held in place with balloon.
Alligator could not grab coil without also grabbing balloon. Balloon deflated and removed. Coil embolized to MCA. Alligator easily advanced into MCA and retrieved coil.
